Title: Ayurvedic approach to erectile dysfunction in diabetes mellitus: A narrative review

Abstract

Background: Diabetes mellitus (DM) is a major metabolic disorder associated with several chronic complications, including erectile dysfunction (ED). Diabetes-associated ED is multifactorial and involves endothelial dysfunction, vascular impairment, autonomic neuropathy, oxidative stress, hormonal alterations and psychological factors. Although conventional therapies are available, treatment response may be inadequate in some patients, highlighting the need for complementary and holistic approaches.

Objective: This narrative review aims to explore the Ayurvedic approach to diabetes mellitus-induced erectile dysfunction and to examine the possible correlation between Ayurvedic concepts and contemporary understanding of diabetic ED.

Materials and Methods: Relevant classical Ayurvedic concepts related to Prameha/Madhumeha, Klaibya, Vata, Meda, Shukra and Ojas were reviewed along with available contemporary literature on diabetic ED and Ayurvedic interventions. Literature concerning Nidana Parivarjana, dietary and lifestyle measures, Shamana, Shodhana, Rasayana and Vajikarana approaches was considered.

Discussion: Diabetes-associated ED may be conceptually understood through Madhumeha-associated Daurbalya, Vata-vaigunya, Meda-dushti, Shukra-dushti and Ojas-kshaya. Ayurvedic management emphasizes correction of causative factors, improvement of metabolic health, appropriate lifestyle modification and individualized therapeutic interventions. Various Rasayana and Vajikarana drugs, including Ashwagandha, Shilajatu, Gokshura, Musali and Ashvattha, have demonstrated potential metabolic, antioxidant or sexual-health benefits; however, direct clinical evidence in diabetic ED remains limited.

Conclusion: Ayurveda provides a holistic framework for addressing diabetes-associated ED by targeting metabolic, vascular, neurological, reproductive and psychological factors. However, standardized formulations, well-designed clinical trials and objective outcome measures are required to establish its efficacy and safety.
